First 2 hours
- Confirm restrictions (stairs, transfers, bathing, equipment)
- Medication reality check: new vs stopped; set a simple schedule
- Hydration + simple food; don’t rush
- Bathroom plan: safe path and timing
Day 1
- Keep the day quiet; avoid too many visitors
- Identify highest-risk moments: shower, transfers, nighttime
- Assign roles: meds, meals, bathing/transfers, transport, overnight safety
Day 2
- Convert “hero mode” into a repeatable schedule
- Write what’s working/not: pain timing, appetite, sleep, mobility confidence
- Confirm follow-ups + transportation
Red flags (call clinician)
- Repeated near-falls, new confusion, severe weakness
- Shortness of breath, chest pain, uncontrolled pain
- Medication side effects making walking unsafe
